Malaysia's Malaysia My Second Home (MM2H) program is Southeast Asia's most established long-stay retirement framework. English is widely spoken (former British colony), healthcare in Kuala Lumpur and Penang is excellent and affordable, food is world-class, and the time-zone-friendly location makes it a hub for Asian travel. Penang is the most popular expat city.

Residents are taxed on Malaysian-source income at 0–30%. Foreign-source income remitted to Malaysia is generally tax-exempt for individuals through 2026 (extended from earlier deadline). Pensions and Social Security from abroad typically not taxed in Malaysia. No wealth tax.
Private hospitals in KL and Penang are JCI-accredited and excellent (Gleneagles, Pantai, Sunway Medical). Costs are 30–50% of US/Singapore prices. Public system not generally available to MM2H holders. International insurance runs $100–300/month.
